We are happy to process your insurance claims to maximize your benefits.  We accept PPO insurance plans.  Insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts.

Payment Options

PAYMENT IS DUE THE DAY SERVICES ARE RENDERED

We gladly accept:  Cash, Checks,  American Express,  Mastercard,  Visa,  Discover,  and Carecredit (with approved credit)

Returned Check

Checks that are returned will be subjected to a $25.00 charge.  We reserve the right to send any NSF checks to the District Attorney's Office.

Missed Appointment

Appointments are reserved exclusively for you.  We reserve the right to charge a $35.00 fee for broken appointments without a 24 hour cancellation notice.  Excessive missed appointments may result in a dismissal from our practice.
